Content: This item is a medical register for patients admitted to an unnamed military hospital between November 1941 and April 1942. At the beginning of each month, a calculation of the total number of patients is given, along with a list of the officers in charge of collecting patient information each week, and instructions for the form and nature of information to be recorded. Patient information is recorded in vertical columns, four to a page, with each column divided into sections recording the patient's name, unit number, date of illness and admission, and any notes on treatment. The bottom of each section is stamped in ink by the officer on duty for the week. Folded inside the latter half of the log book is a slip of paper recording the medical examination of a 2nd Class Private Seizô ITÔ , who was diagnosed with a stomach complaint on April 10, 1944. While there is no ATIS information regarding the date or location of capture for this item, it is probably related to item 1/2/500054/3, which was captured at Finschhafen in April 1944.
