Content: This item consists of blank copies of survey forms for collecting information on Japan's military activities between 1937 and 1945. These forms were produced and sent by the Demobilisation Bureau in Tokyo presumably under the orders of GHQ. It seems likely that these forms were distributed to servicemen prior to their repatriation and submitted to the Demobilisation Bureau upon their arrival in Japan. Information collected include histories of major ground units, service records of commissioned officers and the situation in the area where each surveyee was located after the surrender. Heads of units were also requested to provide information on personnel affairs, weapons and ammunition, treatment of foreign collaborators and local civilians, command structure, preparations and results of operations, provisions and hygiene, and activities between surrender and repatriation.
