Content: This item is a book of field hospital forms recording the admission and treatment of sick and injured soldiers from the 237th Infantry Regiment 1st Battalion between September 1943 and February 1944. At the front of the book is a list of instructions for admission procedures and the correct way to log the patient's symptoms. Each form is divided into columns recording the soldier's name, the nature and date of his complaint, and any notes on treatment. Many of the records contain notes recording the transfer of the soldier in question to other locations in Palau and Manila. A number of different stamps were used to record medical decisions such as "examination," "treatment" and "discharge," but there are no personal seals used by the attending medical officers. This item was captured at Bogia in June 1944.
